New Appointments 

(Please read instructions carefully before booking)

  1. All New appointments are Face to Face.

  2. Please use your child's details when booking, not your own.

  3. Depending on type of appointment, you will need to complete 1 or 2 detailed forms. Details in confirmation email.

  4. ADHD Foundation referrals please select ADHD Foundation appointment. G-PAF questionnaire will need completing /returning before the appointment. If re-direction doesn’t work there is a link in confirmation email.

  5. Focus Mental Health referrals please select FMH appointment. No need to complete questionnaire.

  6. ADHD Diagnosed Elsewhere (Now considering medication) please select New ADHD Medication. Please email the full diagnostic report before the appointment. No need to complete questionnaire.

  7. Self-Referral please select New ADHD Assessment. Please contact me before booking. QbTest report, Parent (PAF) and Teacher (TAF) questionnaires will need completing / returning before first appointment.

  8. Autism Screen. Please contact me before booking. I will send an online link for various forms to be completed before appointment.

  9. Select Liverpool or Birmingham Clinic

  10. The Birmingham Clinic is only occasional Saturdays. If you would like to attend but no availability please email.

  1. All review appointments are video but face to face on request and need.

  2. Select Medication or General Review

  3. ADHD Medication Review (£150 : 30 mins).

  4. General Review (£250 : 45mins). For all general review patients eg developmental, or any long complex discussions. Can also be used for meetings with both parent and other professionals eg teacher, therapist etc.

  5. For F2F review appointment please contact me directly.

  6. After booking appointment then please check you have received a confirmation email.

Review / Follow up Appointment

Prescription Request

Please request a prescription by using the booking system below. It is not an appointment just a way of logging a request so that I can track the request and manage workload. I can only do so many per day.

In ‘Reason for Visit’ section please write Brand and Dose required. Further information regarding medication LINK

Prescription will then be posted out via special delivery within 48hrs (working days) of the date booked.

You will get a confirmation email.

Please note: I will only issue further prescriptions if review appointments are kept to schedule. Please refer to last clinic letter.

Any medication queries will need to be addressed via a medication review appointment please.
